Progress Sheet for Parent Communication

This sheet is used with Comprehension Leveled Readers as a communication tool and incentive for students to read at home.

The mentor and student fill the sheet out together and it is sent home with the book. The child is to read the book at home and have a guardian sign and return the sheet. The student will earn a raffle ticket. At the end of every nine weeks a name will be drawn to win a pizza party during lunch with a friend.

Project MORE Progress

Here is a  progress form that reflects Comprehension Leveled Readers.

The Excel document is what I use to show my administration student growth over the course of the year. In the DIBELS chart I record what reading level was tested, the benchmarks, and actual wpm. There is also a bar graph where I average the cold, warm, and hot reads for each student at every level, and put how many books were read on that level. There is also a place for some of the general demograhpic information, and comments.
